Comcast (phone and internet service)

12:11 PM 8/11/2009
Talked with national phone number...Comcast can provide buiness class service for internet and phone.
They can bring in up to eight phone lines.
Talked with Melissa Pierce local office 419-6624
Talked with Field Tech Greg Beers 846-2412(o) 316-1893(c)
2" conduit with sweeps, 150 feet straight into mech room is fine
CVPS trench specs are fine including the side-by-side cross section
All OK. One combined internet/voip breakout box in mech room.
Greg will check make-ready in September once he finds out what existing CVPS pole will be the jump off point.
Comcast awaiting CVPS to get pole initiated, then can contact CVPS for pole info

04:15 AM 9/14/2009
Called Greg Beers to alert him to CVPS pole, etc.

04:15 AM 11/13/2009
Lee Myers, Comcast Sales, looked at the church building, mech room, etc,
and will forward the information to the commercial sales, Alana Jones.

04:15 AM 11/13/2009
Alana Jones 579-0000, called, Greg Beers will do survey as to what needs to be done.
Alana will call back when that's complete. Survey by 12/09. First $3500
is covered by Comcast, but will be much less anyway. Business class service
which is 4 hour TAT. Pricing will be ~$170 per month for 3 phone lines,
16Mbps internet, and static IP address. Unlimited long distance calling.
Static IP address is $15 per month. 8Mbps vs. 16Mbps is a $5 differential.
Jan 1 is a promotional milestone to understand benefits and tradeoffs.

3:00 PM 12/03/09
Greg Beers visited the site, looked at the mech room location. No issues.
Expecting a call back from Alana Jones now that site visit is complete.

8:50 AM 12/07/09
Called Alana Jones, 579-0000, for installation status, left voicemail.

10:10 AM 12/08/09
Alana Jones, 579-0000, called. She has not heard back yet from Greg. She
should know by Friday 12/12. I'm concerned that build/construction might
stop at some point in December and that we'd have to do temporary DSL until
the spring.
I called/voicemailed Field Tech Greg Beers directly to his cell. 846-2412(o) 316-1893(c)
and asked about winter prognosis for construction.
Alana called back. She needs scans of Vermont Tax ID form, and Fairpoint
phone bill that shows customer name and billing address. The 849-9341 phone
number may or may not be able to be ported. Three phone lines plus
6/1 internet is $155 per month. Static IP address probably not necessary
and we can add it later if desired. Alana will email the forms for initiating
service.
Greg Beers called back. No problem with any tech things. Should be a straightforward
installation and feasible in December. Simply awaiting business things. I
don't have Alana's email yet.

3:32 PM 12/09/09
Alana emailed the 3-page form. I filled it out, scanned it, and sent it back.
I sent the Vermont tax exempt form scan to Alana. Three new phone lines plus
6/1 internet is $155 per month. We're not porting the 849 number. Expecting a
call within five business days (i.e. by Wednesday 12/16) to schedule installation.

10:00 AM 12/14/09

Fairpoint (single phone line for alarm system)

3:18 PM 8/11/2009
Mary Groom 863-0881
Cathy McGrath 658-7132 field tech
2" conduit with sweeps, 150 feet straight into mech room is fine
CVPS trench specs are fine including the side-by-side cross section
likely a 6-pair cable
awaiting nothing from Fairpoint
I'm not thinking we'll use Fairpoint at all. However, we'll run a conduit
for future Fairpoint or other utility. (We found out the state requires
a Fairpoint dedicated line for the alarm system, COLR stuff, etc.)

3:24 PM 9/30/2009
Contacted Mary Groom again regarding our alarm line. She forwarded to
person within Fairpoint to initiate future service: Cathy McGrath 658-7132.

2:02 PM 11/24/09
Talked to Cathy McGrath. Next step...I need to call the Business Office at
866-984-3001.

8:35 AM 11/25/09
Talked to Tina at 866-984-3001. She put RGC "into their system." She will
call back this afternoon. She didn't call back.

11:24 AM 11/30/09
Talked to Allison at 866-984-3001. Gave her the Vermont Tax ID number but
apparently Fairpoint needs the Federal Tax ID number. RGC was not found in
their system. She is sending the request to Tina but I didn't hear from Tina.

3:02 PM 12/01/09
Haven't heard from Tina. Talked to Dan at 866-984-3001. Dan checked with Tina.
A "technician" is dealing with setting up an address because Fairpoint
couldn't find it. I gave Dan the Milton mailing address and the Fed tax ID number.
Dan believes it should be resolved by Friday 12/05 at which point an install
appointment could be made.

4:30 PM 12/03/09
Tina called with order number "1-1 633 038 621". Address has now been set up.

9:10 AM 12/04/09
"499" form sent to engineer by Tina yesterday afternoon who will determine what needs to be done.
Patty Labelle will call call in the next day or two to establish an installation date
which looks like installation could be 1.5 weeks out.
New order number is: "1-1 630 976 502"

11:07 AM 12/08/09
Talked to Jen at 866-984-3001. Installation department, Roxanne, 866-534-2950.
Person who handles that will call me back.
Patty Labelle called. 866-534-2950. She will ping Cathy Mcgrath to ensure that all is
complete on the engineering side. Patty will call back by Thursday 12/17 but call her
back if she doesn't call by that time. Mid-January should not be an issue at all.

9:00 AM 12/17/09
*** REMEMBER: Paul needs to disconnect 849 phone number from his service. ***

9:13 AM 12/21/09
Contacted Patty Labelle, 866-534-2950. Installation on Tuesday 12/29 during
the afternoon.

4:30 PM 12/29/09
Fairpoint phone line installed. Tech needs to return to ground the
system when we have the #6 ground wire installed to the phone board.
*** REMINDER: call/page Fairpoint to ground phone line ***

CVPS

2:58 PM 8/11/2009
Contacted Taryn
initiated Work request # 197717

10:04 AM 8/13/2009
Duane called, 527-6348 x150
First thing is to set pole
Field Tech, Rose Dubois, does line extensions
Rose will call me back today or tomorrow
Fairpoint sets the poles and is the unknown.
From my meter reader, CVPS has been know to simply get the pole from Fairpoint and set it themselves.
We need to determine when we can start so that we can get power in asap after closing.
awaiting call from Field Tech

9:53 AM 8/14/2009
Rose Dubois called, Field Tech, 527-6348 x190
BASIC SCENARIO:
1) Rose will meet with us at site on Monday 8/24 8:30am
2) Application and $172 fee needs to be filed prior to setting pole
3) After the pole is set RGC will do a "contract" with CVPS which includes easement (CVPS does title search.)
4) Then the service extension can be run (overhead cable.)
5) That can be connected to a temporary service at the CVPS pole, or if the underground conduit is ready, at the building.
6) Real or temporary service must have a state Energizing Permit (filed by Mark Smith) prior to meter installation.
COST:
* Approximately $2000 (based on actual distances, hiring of traffic control, anchors, etc.)
TAT:
* Fairpoint sets poles in Georgia.
* Fairpoint is two months out on requests.
* The only way to expedite is if we're not intending to use Fairpoint for phone service. (We're not!)
* If not Fairpoint phone service, then CVPS sets it's own pole (rather than "joint construction" with Fairpoint.)
* If we send application and $172 on August 31, CVPS should (they hope) be able to have service to us by 3rd week in Sept.
NEXT STEP:
* Meet with Rose at site, prepare paperwork

In general, if we really need power sooner than that, a temporary service can be done, complete with
our own pole (3x 2x6 spiked together and braced.) CVPS simply connects to it for a small fee. Short TAT.

Mark Smith notified, all OK with him. His first interaction may be to pull state Energizing permit.
Pratt and Smith may have temp services they'll rent to us; Mark can arrange.
James' contact, Joel, same.

10:04 AM 8/24/2009
Met with Rose Dubois. She will do the pole easement this week while the two
adjoining parcels are still under one owner, only one easement would be
necessary.

Filled out application. Paul hand carried to CVPS in St. Albans. Paul
will leave a copy of a section of the Rugianno site layout at CVPS that
shows suggested pole location, sidewalk, etc.

Alcove in building OK for meter.

10:35 AM 8/25/2009
Rose has the easement prepared. Brett will have has lawyer review and then
sign if all is well. She will send the easement pdf to Brett's lawyer
(Matt Daly, 658-6665.)
The contract is ready for RGC to sign. She will send to Paul. We can sign
when we own the property. Pole and anchor are staked already for our review.
Matt Daly said he'd look at the easement and give me an estimate.

04:00 PM 8/27/2009
Rick Wood covering, Field Tech, 527-6348 x101
Matt Daly not called back yet. Rose out on Friday.

09:00 AM 8/28/2009
Rick Wood covering, Field Tech, 527-6348 x101, pager 742-3128
In actuality, if pole and anchor are on RGC property, then easement
with RGC is a necessity, easment with Brett is a niceity. So I'm scrapping
the idea of a single easement prior to property sale with Brett's lawyer. RGC
will do just a single easement after the closing. Meanwhile Paul is returning
signed contract and payment to CVPS.

09:00 AM 8/31/2009
Ultimately no response from Matt. Rose said that one easement can't work
at this late date. Paul has the easement for Brett's side of the property
and will attempt to get that signed at the closing. A second easement will
be required for the RGC side of the property but that won't gate work because
Rose will assume that RGC in good faith will sign an easement that refers to
book/page at the town office after the deed is recorded, perhaps a month from
now. We could move the pole location(s) but asthetics and tech reasons make
that not a good solution. We'll know more after the closing today and
the contract may need to be rewritten tomorrow (and signed and paid.)

09:00 AM 9/01/2009
On the business side: We closed on the property on Monday afternoon. Pole
and anchor location look good. Brett Brosseau has the easement and will sign
it. Paul will pursue that today. CVPS will write up an updated contract
that includes both easements. Paul will deliver Brett's easement, sign
our easement and the contract, and pay CVPS.
On the tech side: temporary service will likely be at the pole.

Brett was emailed this morning the request to sign and return the easement.
Rose will mail the updated contract to Paul today.
I need to get the Temporary Service application filed with Rose, and
state energizing permit started with Mark.

09:00 AM 9/03/2009
We got the contract and easement in the mail, signed, notarized. Waiting
for Brett's easement and will hand carry. Brett attempted to get his
easement notarized today, will try again tomorrow.

09:00 AM 9/04/2009
Rose is out of the office today. It's likely that
Rick Wood is covering, Field Tech, 527-6348 x101, pager 742-3128.
Pole can be scheduled as soon as contract is received, potentially
ultimately gated by lack of easements.

10:30 AM 9/08/2009
Paul delievered easement, temporary service permit, and payment this morning.
Brett will sign the easement today. Rose met him at his Bank Street
office today to notarize it and be done with it. James said that excavator
will cut driveway at some point but we don't know when. We'll need driveway
and trimming under the line, plus clear the piles in the vicinity of the pole.
I need to keep Rose posted on the access. If we do a final service on the
first framed side of the building then we don't need the temporary service
permit. Still need an energizing permit. Digsafe called by Rose.

10:30 AM 9/10/2009
Asked Rose what trimming is required for CVPS to run line. We don't want
to cut driveway access yet but we're willing to trim for over-street line.
CVPS truck and trailer will have access via existing access driveway and
dirt piles are not in the way (they can drive around them.) Our half (ten
feet) was cleared today. Paul will get Brett's permission to let us clear
his ten feet. Need to let Rose know when we are completely ready. Need
to alert Comcast.

10:30 AM 9/11/2009
Still awaiting Brett's OK to clear ten feet under CVPS line. Mossey cleared
dirt piles away from pole today.

10:30 AM 9/12/2009
Tree in our side of the ten foot swath removed today. Still not heard back
from Brett regarding his side of the ten foot swath.

12:15 AM 9/14/2009
Brett responded. I cut back his side, notified Rose that we're ready to go.
Rose moved the job into the "conctruction" phase.
Contrary to CVPS drawing #203, Rose said that they DON'T inspect the trench.
Called Digsafe, can excavate after 12:15pm on Wednesday 9/16. Ticket #20093801786

02:30 PM 9/15/2009
CVPS set the pole and anchor.

02:30 PM 9/16/2009
Trench dug, 1x 3" and 2x 2" conduits installed, awaiting concrete cover for
3" under driveway (future town road). Rose will have pole bracket installed
tomorrow to show us where to bring sweep up at base of pole.

02:30 PM 9/17/2009
Bracket installed, but on driveway side of pole. Called Rose to alert her.

02:30 PM 9/21/2009
Rose will have bracket installed on the other side of the pole, as it
should have been. Mark thinks he'll pull cable tomorrow, energize on Friday.

9:30 PM 9/22/2009
Pole bracket was removed. Electrical inspector said that fire alarm phone
service has to be Fairpoint ("carrier of last resort?") I called Comcast
(Melissa Pierce) to inquire.

9:30 PM 9/28/2009
Temporary power connected! Rose thanked.

11:54 AM 12/08/09
Rose said that temporary service fee and traffic control fee will be refunded
because we didn't have the classic temporary service and because traffic
control wasn't needed. $404 refund, expedited by Rose.

7:01 PM 01/17/10
Asked Rose what will be required by CVPS from the state (inpsection/forms)
before service can be disconnected, switched, and reconnected.

8:41 AM 01/18/10
Rose said that another energizing permit was needed. To avoid power
being off for too long, Mark will need to coordinate with the state
inspector and CVPS. Schedule the disconnect around 8:00 - 8:30am call
when ready to reconnect and before 2pm. An energizing permit is necessary
from the state inspector unless he says we don't need one and contacts
CVPS to let them know. Hand the energizing permit to the
line crew. Select a good weather day.

11:42 AM 02/09/10
Rose set up Thursday morning for change over to permanent panel. They
will pull the meter on Thursday 8am.

11:30 AM 02/12/10
CVPS pulled meter, panel was switched, meter reinstalled. Done!

Vermont Gas

3:24 PM 8/11/2009
Mark Bergeron 863-4511 x303
http://www.vermontgas.com/commercial/comm_new.html
http://www.vermontgas.com/contractor/
Mark called back. Needs total heat load (BTUs) from Bob Avonda, and then he'll work with Bob and James to determine location of
meter so that Bob can route pipes under slab.
Awaiting Bob Avonda info.
Can't do anything until sided framing exists...something to which they can
attach the meter and regulator.

3:24 PM 10/01/2009
Mark Bergeron visited, initiated forms, sited meter and regulator in
corner of porch. Said we'd need an asphalt box for the tee in the middle
of our driveway to give future access to the locator wire.

3:24 PM 10/05/2009
Tried to reach Lee Brown but only voicemail. Left messages everywhere that
we needed an asphalt box or that Vermont Gas would have to install in
our packed gravel later. To no avail. Let Mark know that we were doing
sub-base stuff today and that we'd give a few days notice before aspahlting
the driveway area.

3:24 PM 11/03/2009
Underground service was installed to building. Asphalt box was installed
and driveway was paved.

8:00 AM 12/01/09
Meter installed today. Waiting for HVAC to be complete to turn on with
HVAC people present. Temporary power will be needed to do that.
Temporary power probably done on Saturday 12/05.

3:00 PM 12/16/09
All four furnaces working on temporary power.
